A client needed a survey and reporting tool to serve to their network of customer websites.
It needed to be highly configurable as each of their customers had individual needs.
At it's core, it can be embedded as a widget into any website and it will adjust to that webisites look and feel if it's using twitter bootstrap.
Once a survey is completed, it will return a report based on the users responses.
Each customer can have a unique setup depending on their needs.
The survey questions can also be unique or shared among sevderal websites.
